Rite Aide Vaccine Clinic (SHOT #2 ONLY)

Residents and community members (16 and older) who attended the City's Community-wide Vaccine Clinic are scheduled to receive their second dose of the Pfizer vaccine on Thursday, May 6, 2021, between 10 AM - 1 PM at the Community Center (2400 Robina Ave, Berkley, MI 48072). 

Rite Aid will be providing shots to those who participated in the first clinic ONLY. 

• Those who received their first shot should show up at the SAME APPOINTMENT TIME they scheduled for during the first clinic. 
• If you received your shot at Rite-Aid, please come around 11 AM 
• If you were an (extra-dose) walk-in, please come around 12 - 12:30 PM

Attendees will need to provide the following materials when they arrive at the Community Center:

• COVID-19 Vaccine Card 
• A completed questionnaire (click here for a copy of the form)

As a reminder, after the injection, participants will have to stay in the seating area for 15 minutes to be observed for any side effects before leaving.
